一群醫學院實習醫生某晚在醫院附近酒吧玩樂時捉弄具有偷窺癖的醫院清潔工肯尼斯（安德魯-李?波茨 Andrew-Lee Potts 飾），並戲虐他為“瘋狗”。被逼飲酒過量的肯尼斯癲癇發作昏厥，成為植物人，就要被終止治療實施安樂死。倍感內疚的實習醫生凱薩琳（阿莉爾?凱貝爾 Arielle Kebbel 飾）違反章程給肯尼斯注射了某種還在實驗階段的混合藥物，試圖讓肯尼斯盡早蘇醒，卻沒想到藥物竟促成了肯尼斯靈魂出竅，附體到他人身上開始殘忍的複仇行動。等到凱薩琳察覺出問題所在，卻為時已晚，肯尼斯的靈魂早已脫離掌控，不管是陌生人還是朋友，甚至包括自己，凱薩琳還能相信誰？
